Are you interested in chiropractic care for yourself or a family member?

Please contact the office to schedule an initial exam. Our hours are flexible and we will work with you to best outline a plan of care.

Coming to the office for your initial exam?

Your appointment will begin with a consultation and review of your health history. You will learn about the importance of a healthy nervous system to your body's overall health and how chiropractic can imporve your body's performance. Following this, certain specific diagnostic procedures will be performed in addition to a full chiropractic exam.

Save time and prepare these forms before you come into the office. You will be able to spend more time with the doctor.

Pediatric Health History (2 pages)
